Everything you need before your first visit.
We welcome new patients of any age. Here's the paperwork, how insurance and payment work, and the answers to common questions.
Patient forms.
Print these and bring them with you. The office can tell you which ones you'll need.
Insurance.
We're in network with several dental insurance companies. Our financial policy lists Delta Dental (Premier Providers), American Dental Plan of Wisconsin, Ameritas, Humana and WEA.
Plans change, so call 608-837-7842 to verify your coverage. As a courtesy, we'll submit your claim for you and give you an estimate of your expected reimbursement. Your portion is due at the time of treatment unless other arrangements have been made.
Paying for your care.
We accept cash, checks, debit cards and major credit cards.
Paying in full
Patients without insurance benefits save 5% when they pay in full by cash or check at the time of treatment. Patients 65 or older save 10%.
Two payments
For treatment over $500, you can pay half by cash or check at your first appointment and the rest the next month.
Three card installments
With an established payment history at our office, you can pay in three equal credit card installments over 60 days, with a signed agreement.
CareCredit
We work with CareCredit as a third-party option for monthly financing. How CareCredit works
Appointment policies.
- If you need to change an appointment, we ask for at least 24 hours' notice.
- After two missed appointments or two cancellations without 24 hours' notice, the office may stop scheduling further appointments and may charge a $30 missed appointment fee.
- Adults more than 10 minutes late and children more than 5 minutes late will be asked to reschedule.
